Client Status - Worker's Compensation

A questionnaire completed periodically to see if there is any change in your medical or work status.
A questionnaire completed periodically to see if there is any change in your medical or work status.
This form is to be completed by your treating doctor and lets the attorney  know whether you are totally or partially disabled and whether the disability is related to your injury at work.
A medical release form required by virtually every medical service provider authorizing the disclosure of various forms of treatment to Attorney Sandman's office.
A form for you to complete after seeing a doctor selected by the Worker's Compensation insurer, the Department of Industrial Accidents, or Social Security Administration.
A form requesting information on exactly how you manage your life activities on a day to day basis.
A form to provide any new information about your case, including additional medical visits, return[s] to work, medications used, relapses or improvements experienced, and the like. This form is important for Attorney Sandman in preparing your case to be heard before a judge.
A form required by the Massachusetts Department of Industrial Accidents that is used by Attorney Sandman in preparing for your hearing.
Travel to and from medical service providers is reimbursable in accepted worker's compensation claims. This form allows you to document these expenses for submission to the insurer.
